By signing this waiver, you are giving up certain legal rights, including the right to sue.

1. Acknowledgment of Risk

I understand and acknowledge that participation in activities at Jungle Land Corp. involves inherent risks, including but not limited to:

• Slips, trips, and falls 
• Collisions with other participants 
• Use of play structures and equipment 
• Equipment failure or misuse 
• Negligence of other participants or staff 

I acknowledge that these risks may result in:

• Serious bodily injury 
• Permanent disability 
• Death 
• Property damage or loss 

I voluntarily accept and assume all such risks, both known and unknown.

2. Waiver and Release of Liability

In consideration for being permitted to enter and participate in activities at Jungle Land Corp., I, on my own behalf and on behalf of my:

• children 
• spouse 
• heirs 
• executors 
• administrators 
• legal representatives 

HEREBY WAIVE, RELEASE, AND DISCHARGE Jungle Land Corp., including its:

• owners 
• directors 
• officers 
• employees 
• contractors 
• agents 
• affiliates 

from any and all liability, including liability arising from negligence, for:

• personal injury 
• death 
• property damage 
• theft or loss 

This applies to all claims, whether arising now or in the future.

3. Indemnity Agreement

I agree to indemnify and hold harmless Jungle Land Corp. from any and all claims, demands, losses, damages, or legal costs arising from:

• my participation 
• my child(ren)’s participation 
• any breach of this agreement 

4. Medical Fitness

I confirm that:

• I (and my child/children) am physically fit to participate 
• I have not been advised otherwise by a medical professional 
• I accept full responsibility for any health-related risks 

5. Supervision & Rules

I agree that:

• I am responsible for supervising my child(ren) at all times 
• I will follow all posted rules and staff instructions 
• I will immediately report any injury or hazard to staff 

Failure to follow rules may result in removal from the facility without refund.

6. Emergency Medical Authorization (Minors)

If I am signing on behalf of a minor:

I authorize Jungle Land Corp. to obtain emergency medical treatment if necessary.

I agree to:

• assume all costs of such treatment 
• release Jungle Land Corp. from any liability related to medical care 
• indemnify Jungle Land Corp. for any claims arising from such treatment 

7. Food & Allergy Disclaimer

I acknowledge that:

• Jungle Land strives to maintain a nut-free environment 
• However, this cannot be guaranteed 

I accept all risks related to allergies for myself and my child(ren).

8. Property & Personal Belongings

I acknowledge that Jungle Land Corp. is not responsible for:

• lost 
• stolen 
• or damaged personal items 

9. Governing Law 

This agreement shall be governed by the laws of the Province of Ontario and the applicable laws of Canada.

10. Severability

If any part of this agreement is found to be unenforceable, the remaining portions shall remain in full force and effect.

11. Parent / Guardian Declaration

I confirm that:

• I am the parent or legal guardian of the minor(s) listed 
• I have full authority to sign this agreement on their behalf 
• I agree to indemnify and hold harmless Jungle Land Corp. for any claims arising from my child(ren)’s participation 

12. Acknowledgment & Signature

I confirm that:

• I have read and understood this agreement 
• I understand that I am waiving legal rights 
• I sign this agreement voluntarily 

13. Electronic Signature Consent

I acknowledge and agree that by typing my name and submitting this form electronically:

• I am providing a legally binding electronic signature 
• This electronic signature has the same legal force and effect as a handwritten signature 
• I intend to be fully bound by the terms of this agreement 

I further agree that:

• This agreement may be executed electronically and stored digitally 
• A copy of this waiver may be used as evidence in any legal proceeding 
• I waive any right to dispute the validity or enforceability of this agreement solely because it was executed electronically
